The Center for Adults Living Well @ the Y (CALW), funded by the NYC Department for the Aging (DFTA) is looking for an experienced professional in the field of geriatrics to keep the Senior Centers day-to-day operations vibrant and humming.

The Case Assistance and Benefits Specialist will provide counseling and guidance regarding entitlements and benefits for those 60 and older. Deliver, develop, and maintain a system for information and referral services.

Your typical day or week may include the following essential functions:

• Provide clients/members with case assistance services and advocate for client benefits and services, including Medicare, Medicaid, SCRIE, SNAP, Public Assistance, Medicare Part D, etc.
• Assist clients with information and referral and with the application for benefits, entitlements, and services
• Provide direct services to clients, including crisis intervention and counseling, as needed.
• Serve as liaison between clients and community organizations and providers; make referrals and follow up as needed
• Maintain case notes for case assistance and information and referral services
• Complete required paperwork and update computer records, as needed
• Participate in regular supervisory meetings, training, agency staff development activities, and outside conferences

Minimum Qualifications:

To land this job, you need to have:

• Available to work Monday  Friday: 9:00 am  4:30 pm (35 Hour, 5 Day Week)
• A bachelors degree
• A minimum of 2 years experience in social service benefits and entitlements
• Be computer literate
• Experience working with adults 60 and older
• Must be fully vaccinated against COVID-19 upon commencement of employment
